HEALTH INFORMATION
- Please complete the Health Information Form for all students in your family and return to the Agape Christi office or email to office@agapechristi.com.
WAIVER AND RELEASE FROM LIABILITY
- Please complete the Waiver and Release from Liability for all students in your family and return to the Agape Christi office or email to office@agapechristi.com.
TRANSPORTATION FORM
- Please complete the Parent Request for Transportation Reimbursement, needed for your local school district for transportation reimbursement. Return it to the Agape Christi office or email to office@agapechristi.com.
- *Minneapolis residents do not qualify for this reimbursement and do not need to submit the form.
IMMUNIZATION FORM (IF APPLICABLE)
Please submit an updated immunization record by the start of school, if applicable (required for new families, kindergarteners, 7th graders, and 12th graders unless we have an immunization exemption on file - see below).
- Kindergarten Immunization Letter to Parents
- 7th Grade Immunization Letter to Parents
- Submit a doctor's record or the fillable form to the Agape Christi office or email to office@agapechristi.com.
- If your family has opted to exempt your child from immunizations, please submit the MN Conscientious Exemption to Vaccination. If you need a notary, please contact Rosie Pike. Completed forms may be returned to the office at Agape Christi or emailed to office@agapechristi.com.
MEDICATION AUTHORIZATION FORM (OPTIONAL)
- If you would like to give your child permission to have either OTC or prescription medications administered while at school, please complete and submit the Medication Authorization Form. Medication must be provided by the family. Completed forms may be returned to the office at Agape Christi or emailed to office@agapechristi.com.
